Mert Ney, 20, sat handcuffed with his head down during a brief court appearance on Monday.

A man accused of killing one woman and injuring another in a Sydney CBD stabbing rampage has only been given paracetamol in custody despite a magistrate recommending he be medicated, a court has heard.

Outside court, Mr Khatiz said Mr Ney, 20, was suffering from "a lot of issues", including being unable to sleep at night.Mr Williams further endorsed the recommendation for Mr Ney to be provided mental health assistance while in custody. Mr Ney faces two further charges of intentionally choking a person with recklessness and common assault, which are understood to relate to a domestic violence incident on August 7.

Appearing via audio-visual link from Surry Hills police station on Monday, Mr Ney sat handcuffed and with his head bowed for the entirety of the hearing as Mr Williams made orders for "intimate" and "non-intimate" forensic procedures to be carried out. Mr Williams also ordered a brief of evidence be served by October 15, with Mr Ney next to appear before the court on that date.
