Investigation make take two years!!!! May I offer a suggestion which can quicken the investigation, the companies entire executive team is sentenced to solitary confinement until the problem properly identified. This should help get some fast answers.
Beware of the term “unplanned maintenance” as it is generally a bogus term that tries to hide poor or no maintenance and basically means something broke usually due to negligence!
If this investigation takes several years. 1) you have lazy investigators 2) the company will walk away Scott free or pay way less than they should have!!!
