Autistic woman took her own life following 'systemic failings' in her care, coroner says

An autistic woman took her own life following "systemic failings" in her care, a coroner has concluded.An inquest heard she was discharged from hospital weeks earlier with no clear follow-up plan.

Over a four-year period from 2016, she was taken to A&E 37 times, mostly after taking an overdose, and in 2019 was sectioned under the Mental Health Act. "Her death was contributed to by actions and inactions by clinicians in a care system that was underdeveloped to manage an autistic individual with complex needs," he said.
