Inspectors say 20% of reviews failed to properly judge if 'reasonable steps had been taken to manage the risk of serious harm'.

A team of inspectors examined the way in which probation services carry out reviews and learn from serious crimes committed by offenders under their supervision in the community."Our inspections found that individual reviews were good in parts," he said.

I noticed this when i was in Norwich hospital and it wasn't all the patience fault no further help with the problem just dropped back into society at the end of sentence
