An inquest into the death of Jared Perry at Pontypridd Coroners' Court heard evidence about the events leading up to Perry's death at the Princess of Wales hospital in Bridgend on Sunday, November 3, 2019. The 32-year-old was discovered unresponsive in his cell at HMP Parc Prison, Bridgend, around midday on Wednesday, October 30, 2019.
10 of those witnesses provided statements for the coroner, Graeme Hughes, to read out. Meanwhile, 16 provided evidence remotely or in person. Delays in transferring Perry to a mental health unit The inquest heard how Perry's mental health deteriorated wile in prison. His mother says he "really suffered with his mental health" at HMP Parc.
When answering questions from G4S advocate Emma Zebb regarding how long it should take for someone to be referred to one of these units Dr Marnell said: “You would hope it would be within two weeks." He was carrying out an "accommodation fabric check" which Lee Burridge, the prison's operational manager, explained was meant to take place every day and was a "cursory process".
Coroner Mr Hughes pointed to a G4S statement which was provided by Mr Boon on November 5, 2019. In it, he provided a different explanation as to why he didn't remove the item. The jury heard how Perry didn't commit any previous act of self-harm or expressed any suicidal intentions. Other members of staff didn't consider Perry to be at risk of self-harm in the days leading up to the incident.
He told the jury that he carried out the observation at 11.28am and 11.41am and said he didn't see Perry on CCTV, although, he heard him "shouting and kicking his door" and thus concluded he was still alive.
