Esther Smyth (79), a married mother of four from Navan, Co Meath, died at the Mater Misericordiae University Hospital on October 2, 2022 after becoming unwell following the insertion of an artificial valve in her heart.

The unexpected death of a woman following a routine cardiac procedure at a hospital in Dublin was due to a previously unrecognised complication of the surgery, an inquest had heard.

She said her mother had been a very fit and healthy woman who went for daily walks on the Hill of Tara with their father until she suffered chest pains which had resulted in her needing the TAVI procedure. In reply to questions from the coroner, he admitted the absence of chest pain meant he had not considered that Ms Smyth might be suffering a blockage to her right coronary artery.

Cross-examined by counsel for Ms Smyth’s family, Anita Finucane BL, the consultant said the risk to the patient was “a bit higher” than average. “People should know about this. This should be published. It is not appreciated that this could happen,” said Professor Galvin. Prof Galvin said he had spent up to 30 minutes trying to remove the blockage but had to abandon efforts as it was “like bone” and he risked perforating the vessel which would almost certainly have resulted in the patient’s death.
