A 13-year-old girl who died from critical injuries she inflicted just moments after leaving a mental health appointment in July this year was let down by an under-funded and under-resourced mental health system, Western Australia's chief psychiatrist says.

"There were so many times when the right kind of intervention could have helped and it was not there," she said.

They also informed the mental health advocate they were uncomfortable with being "pushed" to lay a formal complaint with the police to demonstrate to Kate there were "consequences for her actions". Perth emergency departments have seen a 403 per cent increase in self-harm, suicide risk and attempted suicide presentations in under 13s over the past decade.

The number of children aged zero to 17 referred to CAMHS increased by 51 per cent between 2015 and 2019.
