Large sections of a damning report into failings at the Mackay hospital were not properly redacted. It has since been taken offline.

It contained many redacted sentences, but copying and pasting these sections into a particular word-processing document revealed the true text. It was not immediately clear why some of the assessments made by the investigators were supposed to be kept hidden from the public.D’Ath’s office would not comment and instead referred questions to the Mackay Hospital and Health Service, which has been contacted for comment.
