A coroner has called for new inspection and monitoring procedures of mentally ill patients, after medical staff at an exclusive Richmond clinic repeatedly observed a dying woman from the door of her room but failed to detect any problems.

While not making adverse findings against any staff at The Melbourne Clinic, Coroner Phillip Byrne found the care provided to Elizabeth Ewart, 55, was "perhaps not optimal" before recommending more thorough assessments of patients with mental health conditions. An "exhaustive autopsy" failed to determine the cause of Ms Ewart's death, but her family remain angry that her condition was not effectively monitored by staff.
