Nearly three years ago, a train crash occurred at the train tracks in Prescott, Ontario , injuring a crew member and disrupting freight and passenger rail service . The crash site was located about 95 kilometres south of Ottawa. According to the report, the rail traffic controller did not obtain the train's exact location and mistakenly believed that train 149 had already passed the switch.
Due to the controller's diverted attention and complex workload, train 532 was given permission to enter the north main track. As a result, the crew on train 149 attempted to make an emergency stop but failed to brake in time
