Four men were killed on Dec. 14, 2017, amidst conducting work on a transmission tower near Tweed.

The Office of the Chief Coroner’s inquest into the deaths of James Baragar, 39, Jeffery Howes, 26, Darcy Jansen, 26, and Kyle Sharrock, 27, took place virtually earlier this month. It concluded June 14 and deemed all their deaths an “accident.”

A Transportation Safety Board investigation revealed that an improperly secured tool bag hit the rear rotor of the helicopter, causing it to fail entirely and the aircraft became “uncontrollable.” The jury made 13 recommendations to Transportation Canada, including reviewing all of Hydro One’s current helicopter operations. They also put forth several proposals for safety procedures, enhanced inspections, and reviewing and revising the Canadian Aviation regulations.
