Photo provided by Regina Mullen.An investigation into the death of a 24-year-old Kyle Mullen, of Manalapan, who died while training to become a Navy SEAL, has unveiled multiple problems with the grueling training in which he was participating, officials said.

Among its conclusions, the Navy’s investigation identified “failures across multiple systems that led to a number of candidates being at a high risk of serious injury,” including “inadequate oversight, insufficient risk assessment, poor medical command and control…” and “wholly inadequate” medical monitoring and care after Hell Week, according to a statement from Rep. Chris Smith of New Jersey’s 4th District.
