A meta-analysis of clinical isolates reveals that hypermucoviscous Klebsiella pneumoniae (hmKp) strains exhibit a significantly lower prevalence of ESBL production and slightly lower carbapenem resistance compared to non-hmKp strains.
Strains of Klebsiella pneumoniae (K. pneumoniae) that are hypermucoviscous (hmKp) show a considerably lower rate of producing extended-spectrum beta-lactamases (ESBLs) and a slightly lower resistance to carbapenems compared to non-hmKp strains, according to a recent meta-analysis of 2049 clinical isolates. Researchers performed a meta-analysis to investigate the prevalence of ESBL-producing strains and carbapenem-resistant strains among hmKp and non-hmKp clinical isolates.
The hmKp strains were distinguished from non-hypermucoviscous strains using a string test. The meta-analysis found that hmKp strains had a significantly lower prevalence of ESBL-producing strains than non-hmKp strains (pooled odds ratio, 0.26). Although infections caused by CRKP strains are rare, the authors emphasize the importance of considering these risks when treating patients with hmKp infections who have a history of risk factors associated with CRKP. The study was led by Hiroki Namikawa, Department of Medical Education and General Practice, Graduate School of Medicine, Osaka Metropolitan University, Osaka, Japan. The study had some limitations, including searching only three databases (PubMed, Scopus, and Cochrane Library), potentially missing relevant studies from other sources. The inclusion of only English-language articles may have restricted the scope of the analysis. Additionally, the geographical distribution of the studies was predominantly limited to Asia, which may limit the global applicability of the results
