WASHINGTON—U.S. military personnel often lack the training and resources to properly investigate civilian casualties and take steps to prevent future unintended deaths, a congressionally mandated study of the Pentagon said.

The Defense Department “is not adequately organized, trained or equipped to fulfill its current responsibilities for addressing civilian harm,” the report prepared by the Rand Corp. said.
