The death of a 'fantastic' nurse who took medication which matched that from Royal Preston Hospital where she worked has been described as 'tragic and avoidable' by her family.
A two-day inquest held at County Hall in Preston on Monday and Tuesday heard how Katy, a talented hockey player and 'fantastic nurse', had suffered with mental health issues and Post Traumatic Stress Disorder after twice being assaulted by patients at the Royal Preston Hospital, where she worked.The second attack left Katy with a facial fracture. At an appointment with her GP in March 2018 she described how she had been left feeling distressed and upset by the incident.
The inquest heard that through her job Katy would have had access to the medication which caused her death. An investigation by Lancashire Teaching Hospitals NHS Foundation Trust was not able to conclude that the drugs had come from the hospital. However, the vials found in Katy's room matched the batch number of those at the hospital and there had been three instances of medication not being returned while she worked on the critical care unit.
Towards the end of October in 2019, Katy had sought help from the NHS trust's well-being team but faced a lengthy waiting list. Despite this, her family said she had seemed well, and she had been enjoying success with the hockey team she played for alongside her sister Lucy.
