It has always been my experience that 'weak management' is a huge issue in workplaces. Managers want the title and the salary, but when it comes to actually managing people, they 'don't want to get involved'. This, of course, results in those who try to abuse every rule to do so.

Title correction: “A company that hires a progressively minded HR department pays the price” Often these people claiming to be victims of bullying just aren’t up for the punches. Probably suited to work elsewhere. That’s the black and white truth of it.
