Five young detainees used makeshift weapons, including a computer, in an assault last month that left multiple staff members injured, a report finds.

Five of the young people then allegedly turned on the staff guarding them in what was described as an"unprovoked and violent attack" and attempt to escape.

The report said staff acted quickly and secured four of the detainees within about 10 minutes. But one escaped with a set of keys and freed another inmate held in another unit. "I also want to acknowledge the principled decision-making of three young people who were also in the unit where the incident started, and actively chose not to participate," she said.

"In the midst of being attacked by young people, there is evidence of staff demonstrating a high degree of care and restraint in their dealings with these young people," he said.
